Faulty Cards

EUSR have identified that a small number of cards printed in the last few months are faulty. If a card fails in 6 months or fewer from production it may be replaced free of charge by completing the enclosed form.

EUSR have identified that a small number of cards printed in the last few months are faulty. In the majority of cases this has been that the print on the front of the card has faded or rubbed off. We would like to take this opportunity to apologise for any inconvenience that has been caused by the quality of these cards.

Our card printing process has now been amended and we are confident that cards issued in the future will be high quality and durable. As an added benefit and security feature new cards will also bear a semi-transparent EUSR logo across the middle of the card.
If a card has failed within 6 months of production it may be replaced free of charge by completing this form.

  • Please enclose all faulty cards
  • Cards must have been issued by EUSR within the previous 6 months
  • Please ensure the form is completed fully in order to ensure that cards can be returned
  • No payment is required

Please note replacement cards may take up to 28 days from when we receive and validate the request.

Lost and stolen cards must be replaced using the normal card replacement process, details of which can be found here.
